Job Description:

Duties/Responsibilities:

  • Provide crisis intervention and navigation to patients and families with a behavioral health presentation, including but not limited to: a mental health crisis, substance use, requests for controlled substances, mandated reporting, domestic violence, traumatic/life-threatening injuries and/or death.
  • Collaborate with members of the patient’s care team, CFS and McLean, and community providers to coordinate care with the goal of facilitating treatment and discharge planning.
  • Provide clinical and supportive interventions in collaboration with the patient’s care team to facilitate de-escalation and stabilization.
  • Provide consultation to members of the emergency department care team upon request, including outreach and/or follow up as indicated.
  • Provide telephonic support to patients and their families post discharge to ensure access and connection to behavioral health services, identification of any barriers, and assistance with barrier resolution.
  • Participate in the development and management of patient care plans, including assessing utilization drivers, identifying patterns, and presenting clinical recommendations to the multidisciplinary team.
  • Identify and share resources to meet the needs of patients and their families.
  • Provide and maintain up-to-date documentation in the electronic health record for billing requirements, team communication and/or as required by health insurers.
  • Work to enhance knowledge of integrated care, best practice for improving health outcomes and patient experience, and decreasing health care costs toward achievement of triple aim.
  • Participate in supervision, team meetings, clinical huddles and care planning.
  • Provide debriefing and support to members of the care team, as needed.
  • Adhere to National Association of Social Work ethics and scope of service standards for Social Work Case Management practice.

Education/Experience Required:

  • Masters in Social Work from an accredited program
  • Experience in crisis intervention in acute care medical settings, including de-escalation and debriefing skills
  • Experience in providing patient-centered engagement, intervention and care
  • Experience working with interdisciplinary teams
  • Experience using an electronic health record

Registration/Certification:

  • Currently licensed in Massachusetts as an LICSW or LCSW (without restriction)
  • Must be eligible for credentialing and contracting with all insurers, including MBHP
